DESCRIPTION:Travel Dates: October 7 & 11 \n\n\n\nExciting news! The Choose Life AGM is happening this October in Niagara Falls!\n\n\n\nApply now and be a leader for your community.\n\n\n\nEach community will receive spaces for two youth participants (must be under 18 years of age)\, accompanied by their guardian and a community coordinator. \n\n\n\nYouth do not need to apply with their coordinator. Applications will be reviewed by the Choose Life team\, and selected applicants will be notified. \n\n\n\n*Please be advised that NAN will be covering hotel and travel costs.* \n\n\n\n\n\n\n\n\nBefore you apply for the Choose Life AGM\, make sure you have all the necessary information:\n\n\n\n\n\n\n\nYour Personal Information \n\n\n\nLegal Guardian Contact Information \n\n\n\nGuardian to Sign \n\n\n\nA written paragraph for the following questions: \n\n\n\nWhat do you know about the Choose Life Program? \n\n\n\nHave you taken part in the Choose Life Program in your Community and how has it impacted you? \n\n\n\nWhy do you want to attend the Choose Life AGM in Niagara Falls? \n\n\n\nIf you are chosen to attend\, what information/feedback would you like to share with the Choose Life Program? \n\n\n\n\n \n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nSubmit Your Application:Registration closes on August 23\, 2024.\n\n\n\n\nYouth Application\n\n\n\nChoose Life Coordinator Application\n\n\n\n\n \n\n\n\nPlease direct questions to Shannon Furioso at sfurioso@nan.ca. \n\n\n\n\n\n\n\n\nWe would like to remind everyone that this is a sober event with a zero-tolerance policy for alcohol and drugs.
